Registered Nurse Case Manager for DDS/BIS Residential Homes

Shift: Mon-Fri 8a-4p 40 hours/week

No evenings, weekends or holidays (except infrequent on-call coverage for medical questions)

**$2,000 Bonus Eligible for new hires**


Key job criteria/responsibilities/standards:

Serve as primary contact person and coordinator of medical supports for all assigned individuals.
Perform heath care assessments and develop written health care plans.
Consult with program staff on specific medical interventions, techniques and documentation requirements.

Current Registered Nurse license with at least one year experience. Some experience with people with developmental challenges preferred.
Valid driver’s license.
