Position Summary
The Registered Nurse Home Care is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, therapy, social work, home care aide, home medical equipment and home infusion services as needed. The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care. Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode management.

Primary Job Responsibilities and Accountabilities
•Works at the highest level of licensure by providing quality competent care, following best practice standards, working to achieve care goals and quality outcomes, and following the standards of care set by the state board of nursing.
•Provides effective leadership in the delivery of quality nursing care by assessing, planning, organizing, and directing the nursing functions and collaborating with the patient to implement their plan of care.
•Maintains confidential status of medical records and maintains documentation standards with accurate, timely, and complete recording and reporting.
•Acts autonomously as a vital component of the care team while delegating appropriate responsibility to the skill level of the employee and identifies patient care needs to accomplish a high standard of care in collaboration with the interdisciplinary team functioning as an advocate for those we serve.
•Creates a compassionate, service-oriented environment by following the nursing process in providing emotional, psychological and spiritual support to patients and caregivers.
•Provides a learning atmosphere by sharing knowledge with coworkers/orientation of new staff and by educating patients and their families to meet their health needs.
•Participates in the quality and process improvements at OSF to attain clinical excellence.
•Maintains a safe and clean working environment and complies with policies, procedures and protocols put in place to protect patients and Mission Partners.
•Enhances professional growth and development by participating in and utilizing current evidence based information and keeps abreast of state and national accreditation, regulatory and safety standards.
•Advocates for patients and their caregivers to positively influence value driven and effective nursing care within the healthcare continuum.
•Promotes financial stewardship by assisting with cost reduction without sacrificing quality.

Required
•Current licensure by the State Board of Nursing in appropriate state (Illinois or Michigan)
•OSF Healthcare Nursing Division requires that all employees beginning employment as a Registered Nurse on or after October 1, 2015 are required to earn a BSN degree within six years from Date of Hire and proof of enrollment within your first 18 months of employment.
•Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.
•Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver’s license.

Preferred
•BSN
•Experience as an RN in a healthcare environment

Physical & Mental Requirements and Working Conditions
Uses keyboard, mouse, or other alternative input device. Converses and is understood verbally and in written form. Works in a stationary position for up to 4 hours at a time. Organizes and prioritizes own work as well as the work of others, often multitasking. Frequently works long and irregular hours. Regularly operates a motor vehicle. Occasionally makes presentations, both formal and informal. Responsible for decisions that have a significant business impact to OSF. Hazards include exposure to blood borne pathogens, contagious diseases and chemical exposure.
