Job Description
Tour II ( 8:00AM-4:00PM)

PURPOSE OF POSITION:

Under the supervision of the Chief of Ambulatory Care, the Director of Nursing for Specialty Medical Homes is responsible for supervising the clinical operations of Ambulatory Specialty Care Centers at Kings County with the overarching goal of ensuring our patients receive the care they need to live their healthiest lives. We are transforming to a “Specialty Medical Home” model, which delivers care that simultaneously focuses on optimizing the patient experience as well as clinical outcomes.
Each Specialty Medical Home will have a full-time assigned Nurse Practitioner to support this work. The Director of Nursing will provide direct supervision of these Nurse Practitioners while working with medical leadership to improve the design of the practices to meet our goals.

Center for Liver and Gastrointestinal Disease
Center for Stroke and Neurological Illness
Center for Thyroid and Endocrine Illness
Center for Urologic Illness
Core Elements of Specialty Medical Homes include:
Patient-Centered Design
Advanced access: same day sick visits, telephone/telehealth care
Continuity of care by providers
Electronic consults (“E-consults”) that eliminate unnecessary visits
Evidence-Based Care
Clinical pathways and protocols
Continuous quality improvement ensuring adherence to standards of care
Population Health Methodology
Prevention of hospitalization through identification of high risk patients
Use of disease registries to ensure all patients receive effective care
Screening for and addressing social determinants of health
Core functions of the Director will include:
Collaborate with physician leads to develop and update clinical pathways
Train, supervise and mentor Nurse Practitioners working in Centers as they take on new roles that support patient-centered, high quality care
Team development and leadership
Partner with both the medical, nursing and administrative leads in strategic planning
Provide direct care within one of the Centers (1 day per week)
Typical tasks include:
Attend weekly tiered huddles in each of the Centers
Develop standard work documents for new clinical processes
Train appropriate staff in new workflows
Develop staff and team engagement around new initiatives
Coach individual staff around adopting new work flows as necessary
Monitor implementation
Review and analyze quality metrics and develop improvement plans
Attend departmental, interdepartmental and corporate meetings as assigned
Collaborate with nursing leadership on training activities for nursing personnel
Participate in special projects and performs other duties as indicated

Job Requirements

Minimum Qualifications:
Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.
• A Master's Degree in Nursing Administration, Education or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching or administrative capacity; or,
•A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity; or,
•A satisfactory equivalent combination of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
•BLS certified required
•Prior experience with Performance Improvement/Quality Improvement

Knowledgeable in promotion of patient experience
Excellent teaching skills, able to inspire and motivate staff (staff engagement)
Excellent organizational and management skills
Knowledge of current best practice trends in Ambulatory Care
Proficiency in written and oral communication

Required:
Certified Adult, Geriatric or Family Nurse Practitioner
Experience (minimum 2 years) in a health care leadership position

Preferred:
Experience in performance improvement/quality improvement
Experience in an ambulatory care setting
