We are a small home care agency looking for a clinical manager who has graduated from an accredited school of nursing and is currently licensed to practice in the state of Massachusetts. This position is full-time, Monday through Friday and located in Lexington, MA.

Responsibilities include but are limited to the following:
• Participates in developing standards which ensure safe and therapeutically effective service to clients and families. Has joint responsibility with the Administrator for seeing that standards are met.
• Participates in developing objectives and making sure they are implemented.
• Is responsible for recruiting, hiring, evaluating and terminating service personnel.
• Participates in planning for the orientation of new employees. Conducts selected orientation classes.
• Monitors probationary period for new employees.
• Assures ongoing assessment of client/family needs and implementation of interdisciplinary team plan of care.
• Serves as a role model for all colleagues by setting an example of high standards in dress, conduct,
cooperation and job performance.
• Accepts responsibility for regular attendance and punctuality and fulfills job requirements without regard to time involved.
• Serves as a resource person to employees.
• Attends pertinent continuing education programs other than routine in-services and shares information with staff.
• Oversees Quality Assessment and Performance Improvement (QAPI) program.
• Responsible for overseeing development of indicators with appropriate data collection, aggregation and analysis, taking action and reporting results.
• Part of the 24 hour/day, seven (7) days/week on-call coverage rotation.
• Assures that clients’ plans of care are developed, implemented and updated.
• Conducts client case conferences, in-services, staff meetings and maintains documentation; participates in community education projects.
• Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.
• Makes clients and personnel assignments.
• Ability to deal effectively with high levels of stress.
• Addresses and corrects all client’s concerns.
• Ability to supervise, guide and develop skills and performance of service personnel.
• Ability to exercise independent judgment.
• Ability to work as a team with other individuals.
• Ability to enlist cooperation of many people in furthering a program.
• Ability to assist in evaluating personnel.
• Maintains expected productivity for each staff member.
• Completes field supervisions timely.
• Documents counseling/disciplinary actions.
• Develops working relationship and positive attitude within the team.
• Monitor orientation process of new team member.
• Maintains a schedule for meetings and conferences with staff.
• Oversees the annual mandatory requirements.
• Possess leadership skills and supports agency’s goals and values.
• Case conferences with staff member(s) and client(s).
• Monitor and assure timely completion of incomplete documents.
• Customer service for patient/family problems and complaints.

Job Requirements

• Previous experience in home care
• Two years supervisory experience, but willing to train the right person.
• Organizational skills
• Attention to detail
• Effective/verbal communication skills
• Strong leadership skills
• Able to resolve conflicts, confrontation with courtesy and tact.
• Current nursing license
• Current CPR
• Valid Driver’s license
