Eklego is seeking RN Case Managers for our client's Home Health division. Eklego has partnered with Overlook VNA Services in Charlton, MA, part of Masonic Health Systems, in hiring qualified and compassionate staff. What will I do as an RN Case Manager? This is a direct-hire, full-time position, working Day time hours Monday-Friday with rotating weekends and on-call responsibilities. The Registered Nurse Case Manager works independently throughout Charlton, MA providing initial and ongoing assessment of client's skilled nursing needs. The RN Case Manager will also implement care plans and make referrals to other services that support the patient's care plan. You'll also be provided with all the supplies you'll need for documentation and treatment.

Requires:

Current RN license for the state of MA
Valid Driver's License and reliable vehicle
Preferred 1 year of experience in Home Health Nursing
Current CPR Certification
