Job description
Tallavera is currently seeking a Registered Nurse (RN) Case Manager, in Rochester, NY working in Home Care. Preferred candidates for the Registered Nurse (RN) Case Manager position will be a New York State Licensed Registered Nurse, with 1 year experience as a clinical nurse in a hospital or other direct patient care clinical setting.

Responsibilities of the Registered Nurse (RN) Case Manager:

Assesses and charts observations of the patient’s condition at each visit
Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third-party payors including obtaining medical orders and approval of medical services.
Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.
Requirements of the Registered Nurse (RN) Case Manager:

NYS driver’s license
NYS Licensed Registered Nurse
OASAS certification preferred
