Registered Nurse – Case Management- Case Manager - Travel - (CM RN)

Join our team as a Case Manager Registered Nurse (RN - Case Manager) in Warren, MI. In this role, you will coordinate patient care and support seamless transitions between care levels. You will conduct comprehensive patient assessments, develop personalized care plans, and collaborate with healthcare teams to ensure timely interventions and specialty referrals. Your responsibilities include monitoring patient progress, facilitating education, and implementing strategies to overcome barriers to care. Discharge planning is a key responsibility, requiring you to address physical, social, and financial needs while coordinating with community resources for smooth transitions. You will also participate in quality improvement initiatives and work closely with interdisciplinary teams to optimize patient outcomes. To qualify, you need a Bachelor’s degree in Nursing (BSN) or an Associate’s degree in Nursing (ADN) with at least two years of acute care case management experience. ADN candidates must enroll in an accredited BSN program within three years of hire and complete it within five years. A minimum of three years of recent acute care nursing experience is required, along with strong clinical judgment and excellent communication skills.
