Job Summary:
As the Director of Case Management, you will oversee and coordinate department activities, ensuring
compliance with organizational policies and external regulatory agencies. You will be responsible for
managing all aspects of daily hospital case management operations, integrating patient care and
discharge planning processes, and achieving established outcomes through interdisciplinary
collaboration. Our client is renowned for providing quality healthcare to Northeast Florida and surrounding areas. With 400+ bed acute care hospital features services such as a Chest Pain Center, Stroke Center, dedicated Heart Center, and a state-of-the-art Spine Clinic. Also offering comprehensive maternity services. They are a Certified Level II Trauma Center.
Key Responsibilities:
- Direct and evaluate departmental operations, including staffing, onboarding, and use information technologies.
- Allocate resources to meet productivity and quality goals.
- Oversee core functions and practices of case management.
- Ensure adherence to care coordination and discharge planning processes.
- Collaborate with the Facility CFO and Division CM Leader to achieve goals and expectations.
- Assess and improve department performance through data analysis and sustainable performance improvement activities.
- Ensure compliance with policies and standard operating procedures.
- Establish working relationships with key stakeholders, including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
- Perform other duties as assigned.

- Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
- Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
- 5-7 years’ experience in acute care hospital case management.
- 2+ years’ experience in case management leadership.
- Experience in a leadership role in a large acute care facility, preferably a Trauma Level hospital.
- Strong communication skills, ability to multi-task, and a polished professional demeanor.
