Description

LPN Position in West Chester PA

Perform professional services, as ordered by physician, such as: vital signs, urinary catheterization, tube feeding, applying/changing dressings, packs, colostomy and drainage bags, accuchecks, and stool guiacs.

Regulate and adjust medication and treatments per consultations with physician, monitoring closely for adverse side effects.

Participate in formal and informal resident teaching activities aimed at promoting positive health practices and an understanding of the individual's medical issue(s).

Assist with the ongoing therapeutic management of stable long-term health problems in the chronically ill elderly.

Act as an advocate for the residents when indicated, assist in supporting them in the least restrictive, most independent and yet safe environment.

Coordinate and utilize other community health care resources. Comply with accepted professional standards and principles. Assist in staff development regarding the aging process.

Coordinate and facilitate medical follow-up and implementation of recommended medical care.

Initiate Care Plans and make changes as needed. Participate in Care Conferences as Assist with case management as needed.
Direct the day-to-day functions of CNAs in accordance with current rules, regulations and guidelines that govern long-term care.

Cooperate with other resident services when coordinating nursing services to ensure the resident's total regimen of care is maintained.

Participate in Care Center surveys (inspections) made by authorized government agencies as requested.

Admit, transfer and discharge residents as required.

Complete the nursing portion of the resident's discharge plan.

Complete accident/incident reports as required, and notify Nursing Supervisor. Make shift entry on the nursing 24 Hour Report.

Receive telephone orders from physicians and record on the Physician's Order Form.

Transcribe physician orders to appropriate location (MAR, TAR, Dietary, etc.) and sign off order as “noted” and include date and time noted.

Participate in end of month “RECAPS” activities to ensure accuracy.

Document nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as responses to the care.

Perform routine documentation duties

Prepare and administer medications and treatments as ordered by the physician maintaining compliance with regulatory mandates such as time of administration and med-pass protocols

All additional duties as assigned

Requirements
Valid LPN Licensure
Must be able to read, write, and speak fluent English
Must possess and maintain good physical and mental health
Must demonstrate excellent observational skills, good nursing judgement, and communication skills
